Admission Process

Our admission process ensures families understand our program, expectations, and Wisconsin licensing requirements.

1. Family Handbook
All families will receive a copy of the Magical Moments Childcare Policy Handbook, which details our policies, procedures, hours, fees, health requirements, and daily operations.

2. Meeting With Provider
Parents will meet with the provider to discuss the child’s needs, review policies, and determine an appropriate start date.

3. Required Enrollment Forms
The following forms must be completed and returned by your child’s first day:

• Child Care Enrollment
• Health History & Emergency Care Plan 
• Intake for Child Under 2 Years (if applicable)
• Alternate Arrival/Release Agreement (if applicable)
• Transportation Permission (if applicable)
• Parent/Provider Agreement

These forms must be completed:

• CACFP (Child and Adult Food Program)
• Child Health Report (signed by physician) 30 calendar days
• Immunization record or electronic printout 30 calendar days

An updated immunization record must be kept on file.
Children may not begin care if required forms are not completed within licensing timelines.

Tuition Structure & Payment Policy

Magical Moments Childcare provides:

-Full-time (30–40 hours per week) care.

-Part-time (24–29 hours per week) care.

No child may be enrolled for fewer than 2 days per week unless it's weekend only care.

Tuition is charged weekly and is based on your child’s age group and scheduled hours, as listed on the Magical Moments Rate Sheet in the policy handbook.

Payments are due in advance each Friday for the upcoming week.
Parents are responsible for all fees not covered by third-party payers.

There are no reductions or refunds for absences, illness, holidays, or missed days.

Other Fees

Late Pick-Up Fee:
$5 per 15-minute interval (or any portion thereof) per child for pickups after your scheduled time.

Late Payment Fee:
If payment is even 1 day late, a $5 daily late fee will be added until the balance is paid.

Returned Payment/NSF Fee:
A $25 charge will be added for any returned check or declined transaction.

Registration Fee:
Magical Moments does not currently charge a registration fee unless stated in an individual Parent/Provider Agreement.

Special Tuition Programs & Funding

Wisconsin Shares / County Assistance:
Families receiving WI Shares must pay their required copay by the due date listed in their Parent/Provider Agreement.
Any changes to your authorization must be reported to Magical Moments immediately to avoid interruption in care.

Foster Care:
Foster children are accepted. Financial arrangements may vary depending on county guidelines. Please discuss details during your enrollment meeting.
